DENTAL SAVINGS PLAN

In an effort to encourage good dental health and alleviate rising dental costs, we have developed an In Office “Dental Insurance” Plan for our patients who do not have dental coverage through any third party insurance or find that our plan costs less than their current dental coverage plan. Since nearly all dental insurance plans require group participation to get any type of price reduction, individuals and families seeking dental insurance on their own often find it too expensive to even consider and rarely get a full return of their premium with treatment. Alternatively, our dental savings plan provides benefits for the full scope of treatment at an affordable price to help minimize the cost of your dental needs. It also offers portability; you are not tied to any one employer. This is your plan and it follows you regardless of who you or your spouse works for during the year.

Unlike a dental insurance plan, there is no third-party, no deductibles, copays or waiting periods. Enrolling in our In-House Dental Savings Plan gives you the opportunity to obtain your dental treatment exclusively at our practice for reduced fees. It cannot be combined with dental insurance or other reduced fee dental plans.

adult Programs

Age 15 and over

The Adult Program includes:​

ADULT PROGRAM: $425
  • 2 Exams per year

  • 2 Dental cleanings per year

  • All diagnostic X-Rays

Kids Program
KID'S PROGRAM: $375

Age 3 to 14

The Children's Program includes:​

  • 2 Exams per year

  • 2 cleanings per year

  • 2 Flouride treatments per year

  • All diagnostic X-rays

ENROLLMENT IN EITHER PROGRAM COMES WITH THE ADDED BENEFITS OF:

  • 40% all other Preventative Care, such as Emergency exams, Sealants, Periodontal Maintenance and Mouth Guards
  • 30% all Minor Procedures, such as Fillings, Scaling and Root Planing (Deep Cleanings) and Extractions
  • 20% all Major Procedures, such as Root Canal Therapies, Crowns, Bridges, Space Maintainers and Dentures
  • 10% all Orthodontic Needs, such as Invisalign treatment and Retainers

FINE PRINT:

  • Annual Enrollment Fee for each family member is non-refundable, even if the plan benefits are not fully utilized during the participation period.
  • Payment must be paid in full by the initial enrollment date. 
  • Enrollment Date begins on the first date of use and provides coverage for 1 year to the date.  Enrollment is available on a yearly basis only.
  • Any costs associated with dental treatment is expected to be paid in full.
  • The exams, bitewing x-rays, cleanings, and fluoride must occur within the year of enrollment and cannot be carried over to the next year.
  • It is the patient's/parent's responsibility to make and keep appointments for his/her family members.  A fee (based on length of appointment) may incur for each broken appointment without a 24 hour advance notice.
  • Our program is not transferrable to another party or uncovered family member.
  • Participation cannot be combined with any other offer or dental plan.
  • Payments towards any dental treatment using Care Credit’s extended payment plan, may be subject to a 10% decrease in the discount rate to off-set the merchant fees.
  • All fees shall be based on our practice's UCR (usual, customary and reasonable) fees, which are subject to change, and not any other dental plan's fees.  
  • Annual Enrollment Fees are subject to change.
  • This plan cannot be used in Workman’s Compensation Treatment Plans.
  • As this is an “In House” plan, it’s discounts cannot be applied to any specialist to whom we refer.
  • Benefit coverage is effective for participants who remain enrolled in their plan at least until the completion of treatment.  If enrollment expires before treatment is completed, coverage is no longer available.
